How To Fill Arizona Rental Application Form

  • Section 1: Applicant’s name, marital status, date of birth, phone number, and SSN

  • Section 4: Information about present and previous residences like address, duration of stay, name and phone number of landlord; please provide previous residence information for duration of stay less than two years
  • Section 5: Employment information like name, address, and phone number of employer, designation and duration of employment, monthly income, name and phone number of supervisor along with other sources of income; please provide information of previous employer in case the employment duration is less than two years
  • Section 6: Information about spouse and spouse’s employment like name, SSN, phone number of employer, designation and duration of employment, monthly income, name and phone number of supervisor along with other sources of income; please provide maiden name of spouse if applicable
  • Section 7: Information about bank, account number, account type, and address
  • Section 8: Information about credit references in requested manner
  • Section 9: Information about vehicles owned and driver’s license particulars of the applicant and spouse
  • Section 10: Personal information about eviction, bankruptcy, drug use, and conviction
  • Section 11: Information about warrants if any
  • Section 12: Information about waterbed and waterbed insurance
  • Section 13: Emergency contact information
  • Deposit to Hold Agreement: Amounts of security deposit and fee, along with last date for cancellation
  • Rental Agreement Information: Please provide requested information about rent, security deposits, and tax
  • Space for date and signature of applicant along with management’s acknowledgement
